Rates and Self-pay Options​

  • Diagnostic Assessments - $115-$127

  • 60 Minute Therapy session - $94

  • 45 Minute Therapy session - $68

  • 30 Minute Therapy session - $53

  • Group Therapy Rate - $32

  • Letters - $50/$150+

  • Written reports - $110 p/h

  • Court Related Appearances: $375 per hour (4-hour minimum required; not typically covered by insurance providers)

  • Consultation fees: $25 per 15 minutes (This includes attending parent/teacher conferences, attending school meetings, conducting classroom observations, interacting with insurance providers, and other clinical interactions upon your request)

Please note that copays are determined by your insurance provider. The rates listed are applicable to self pay clients as well as clients whose insurance provider will not cover the entire balance.

Payments

​

You are responsible for paying at the time of your session unless prior arrangements have been made. No subsequent sessions will be scheduled if there is an outstanding balance. We accept many major forms of payment, including credit cards, debit cards, cashier’s checks, money orders, and cash. WE DO NOT ACCEPT PERSONAL CHECKS. Please be aware we charge a $45.00 credit card decline fee if your credit card is declined for any reason. Payment policies will be provided to each client's guardian prior to the first appointment

Cancellation Policy

 

Once an appointment is scheduled, that time is reserved only for you. If you are unable to attend a session, contact your therapist at least 24-hours in advance and there will be no charge. For sessions missed or cancelled with less than 24-hours notice, the regular fee will apply and cannot be billed to insurance providers.

Testimony for Divorce, Custody, and Visitation/Parental Access

Milestones Therapeutic Center does NOT participate in custody hearings, custody/access disputes, visitation, or divorce proceedings.  These matters are not within our scope of practice.  You will need to contact a Parent Coordinator, Guardian ad Litem or another neutral party to assist you in this matter.

Form and Letter Requests

 

From time to time, caregivers may request letters for disability, schools, court, parenting coordinators or other such needs.  A checklist style form provided by the caregiver will be completed by the therapist at a rate of $50.  Any prepared letters requiring narratives for other purposes are assessed at $150 per letter for up to 2 pages. There is an additional fee for greater than 2 pages. For more detailed assessments, please discuss it before or after session. Please note, that insurance often does not cover these fees.

If your insurance provider is not listed or you’re uninsured, I may still be able to work with you. Self-pay clients are welcome and I am currently working on adding Medical Mutual, Molina, Paramount, Aetna, Humana and Humana Military to my provider network. Please contact me or check back regularly for updates.

 

Additionally, many insurance plans provide reimbursement for out-of-network mental health services. Milestones will provide a receipt for services, referred to as a “superbill,” that includes information needed to file for reimbursement with your insurance provider. You may want to consult with your insurance provider as to whether or not your plan provides out-of-network mental health benefits.
